To complete a survey on behalf of a Resident:

  1. Ensure the survey has been assigned to the Resident in Assign Residents.
  2. Click on Surveys in the side nav.
  3. Select Survey Status.
  4. Under Available Surveys, select the chosen survey from the 'Select survey' drop down list.
  5. Click on the Complete button in the Actions column for your chosen Resident.
  6.  The survey will open for completion.  
  7. If the Resident wants a hard copy of the form, click the Download Survey Form from the bottom of the screen.
    Then click on the downloaded PDF file to View and Print the form.

  8. Now begin filling in the survey answers in the online form. The Resident name will populate by default, but if the Resident would prefer to give anonymous feedback, tick the checkbox for answers to remain anonymous.
  9. Complete the survey questions, as relevant.  The star rating system works on a scale of 1 star (poor) to 5 star (outstanding). If you make a mistake when clicking on the stars, click on the minus sign to clear your score:
    Ensure you complete all relevant sections of the survey:
  10. If the Resident is happy with the information entered on their behalf, click Publish. The survey will now have a status of Completed for this Resident.
  11. If you only get part-way through the survey, click Draft.  This enables the answers to be saved and the survey can be completed at a later date for this Resident. The next time the Complete button is clicked for this Resident, these previous saved answers will populate automatically. 
  12. If the Resident changes their mind and no longer wants to have answers submitted on their behalf, click Cancel.
